About this template

Ratings on their own tell you the score and nothing else. This survey pairs the numbers with room to explain them: how satisfied someone is, how likely they are to recommend you, what worked, and what did not.

The written answers are where the useful material sits. Ten people typing the same sentence about your checkout is a clearer instruction than a score moving by half a point, and it arrives with the wording you can quote back to the team that owns it.

Keep it short if you send it after every purchase. The longer version belongs in an annual review, not a receipt email.
