About this template
A demo request that only captures a name and an email guarantees the first call is spent asking questions that could have been answered in a form. Team size, current tooling, and the problem in the prospect's own words are the three answers that let a rep prepare, and none of them cost the prospect much to give.
The timeline question is the one that sorts the pipeline. Somebody who needs this in place this week and somebody who is just researching both deserve a reply, but not the same reply, and asking directly is more reliable than guessing from job title. The must haves checklist doubles as a record of which features actually pull deals in.
Route submissions by team size so enterprise enquiries reach the right person automatically. Add a payment field only if you sell self serve, and keep the problem field required, since it is the one that makes the call worth taking.
