About this template

IT requests arriving as messages cannot be prioritised, because none of them say how urgent they are. This form asks for the category, the item, the quantity, why it is needed, how urgent it is, and whether it replaces something that already exists.

The replacement question is the one that saves money. A broken laptop and an additional laptop are different purchases with different budgets, and the asset tag beside it means the old machine gets wiped and retired instead of living in a drawer.

Add your approval threshold to the cost field, and connect the form to your ticket system so requests land in the same queue as everything else.
