About this template

Tickets that arrive as free text cost an extra round trip before anyone can start work. This form asks for the product area, the severity, what happened, and what the person expected to happen instead.

Severity is the field that earns its place. It sets queue order, and asking the reporter is more reliable than inferring urgency from how strongly the message is worded.

Add a file upload for screenshots and logs. Teams on a paid plan often add an account or order number so the agent opens the right record first.
