About this template
Party bookings taken over the phone lose the allergy list about a third of the time, and that is the one detail with real consequences. Making allergies a required field with help text asking for every allergy in the group, and an instruction to write none, means the kitchen gets an unambiguous answer every time.
Separate counts for children and adults are worth the extra field because they cost different amounts and need different amounts of space. The appointment field only offers slots you are actually free for, which removes the back and forth about a date that was already taken.
The extras checklist is where the margin is, and listing them as options rather than describing them in prose gets a noticeably higher attach rate. Add a payment field for the deposit so the slot is only held once money has moved.
