About this template
A caterer cannot quote from a headcount alone. Service style, course count, and what the venue actually has in the way of a kitchen change the price more than the number of guests does, and all three are asked here before anybody talks about money.
The facilities checklist is the field that prevents the worst kind of surprise, which is arriving to plate sixty covers somewhere with no running water. Ending it with an option saying nothing, we bring everything is what makes people answer honestly rather than optimistically.
Dietary requirements asks for rough numbers alongside the requirements, because four vegetarians and forty are completely different jobs. Add a payment field for the deposit, and make the budget field required if you would rather not quote blind.
