About this template

Referrals are the highest converting source in most hiring funnels and the worst captured. They arrive as a name dropped in a corridor, with no email, no role, and no reason attached. This form takes thirty seconds to fill in and gives recruiting everything it needs to make contact the same day.

How you know them is the field that separates a real referral from a forwarded profile. Somebody who managed the candidate can tell you things no interview will surface, and the answer tells recruiting how hard to lean on the recommendation when they read it.

Ask whether the candidate knows, because a cold approach that opens with a colleague's name lands badly when the colleague never mentioned it. Add a bonus eligibility field if you pay for referrals, and route these straight into your hiring channel so they do not sit in an inbox.
