About this template
Engagement surveys fail for one of two reasons: they are too long to finish, or people do not believe the answers are anonymous. This one is eight scored statements and two open questions, which takes about three minutes, and it asks for department and tenure rather than name so you can still segment the results without identifying anyone.
The statements are deliberately written as agreements rather than questions. Rating how strongly you agree that your workload is manageable produces a cleaner distribution than asking how happy you are with your workload, and it lets you compare the same statement quarter after quarter without rewording anything.
Push results into a spreadsheet and chart each statement over time. If you have fewer than about eight people in a department, merge the department options before you send it, because a team of three is identifiable no matter what you promise.
