About this template
Event feedback has a short half life. Sent within a day, response rates run several times higher than a week later, and the answers are specific rather than a general impression. This form is built to be finished on a phone on the way home, which is why the scored part is five ratings and one scale.
Rating venue, food, and content separately is what makes the result actionable. An event that scores well overall can still be losing people on one dimension, and a single overall score hides exactly the thing you would have fixed. The recommendation scale gives you one number to track between years.
The question about next year's programme quietly doubles as content research, and it is usually the field with the most usable material in it. The quote permission checkbox lets marketing use the good answers without going back to ask, which they otherwise never get round to doing.
