About this template

Exam entries are unforgiving: after the deadline, a wrong paper or a missing access arrangement cannot be fixed. This form asks for the subject, the papers, and the session as separate choices so there is no room for a note in the margin that somebody has to interpret later.

Access arrangements get their own section with an upload for evidence, because that is the part most likely to be handled by email and then lost. Listing the arrangements as checkboxes means the exams officer can filter entries by requirement and book rooms and readers from the same data.

The declaration about the deadline is doing real work, since it is the record that the student was told. Add a fee field if you charge for resits, and use a rule so the evidence upload becomes required whenever an arrangement is ticked.
