About this template

An exit interview held in a meeting room gets you the polite version. Written answers, submitted at home after the last day is already fixed, get you the real one. This form asks for the main reason first as a single choice, then opens the floor, because people will pick a reason honestly long before they will write three paragraphs about it.

The likelihood question is the number to track. One departure tells you very little, but the same score falling across a year of leavers tells you something is wrong that nobody is saying out loud in reviews. Pair it with the two open questions and you get a reason plus a fix, which is the only combination that survives being taken to a leadership meeting.

Route submissions to a shared HR address rather than to the leaving person's manager, since manager relationship is one of the options and nobody will pick it otherwise. Drop the rehire question if you do not run an alumni pipeline.
