About this template
Bringing on a contractor involves a set of details that are awkward to ask for in conversation and expensive to get wrong: the rate basis, the tax status, and how much notice they need. Putting them in a form makes the questions routine rather than personal.
Rate and rate basis are separate fields on purpose. A number on its own is meaningless when some people quote by the hour and others by the day, and a single free text field produces a column that cannot be sorted or compared across a roster of freelancers.
The two confirmations at the end are the ones that prevent the most common contractor dispute, which is an assumption about guaranteed ongoing hours that was never actually agreed. Add a bank details step outside the form, since payment details do not belong in a submissions table.
