About this template
Internship rounds get hundreds of applications and most of the screening time goes on two questions that a form can just ask: when can they actually start, and do they need sponsorship. Putting both up front means the shortlist is built from applications you can genuinely act on.
Asking for a portfolio link alongside the CV upload matters more for interns than for experienced hires, since a student's best evidence is usually a project rather than a job history. The why field is capped by its help text at a few sentences, which improves the answers and makes them comparable.
The team preference dropdown lets you split submissions to different reviewers automatically. Add a question about university term dates if your programme has to fit around them, and drop the graduation date if you only hire final year students.
