About this template
Grooming appointments overrun because the coat was worse than described or the animal would not tolerate handling. Asking about coat condition on a four point scale and about temperament as a checklist lets a groomer book the right length of slot rather than discovering it halfway through.
The temperament options are specific behaviours rather than a general judgement, because no owner describes their dog as difficult but most will happily tick that it does not like its paws touched. That is the answer that actually changes how the appointment is handled.
The two confirmations at the end cover the two disputes that genuinely arise in grooming: a matted coat that has to be clipped short, and emergency veterinary treatment. Both are far easier agreed at booking than argued about afterwards, and the vet field is there so the second one is actionable.
