About this template

Chasing a referee by phone costs two or three attempts and produces notes only the person who made the call can read. Sending a link means the referee answers when it suits them, in writing, and every candidate gets asked the same six questions so you can actually compare the answers side by side.

The working relationship question is the one that does the most work. A glowing reference from a peer means something different from the same words written by the person who managed them, and knowing which you are reading changes how much weight it carries. The duration field is there for the same reason.

Keep the final question broad. Asking whether there is anything you should know before making an offer gives a referee permission to raise something they would never volunteer against a narrower prompt. Add a role specific question if you are hiring for something regulated.

Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.
