About this template
Choosing between three applicants means comparing them, and you cannot compare what arrived in three different formats. This form asks every applicant the same things: current address, employment type, gross monthly income, who else will live there, whether there are pets, and the previous landlord's number.
The consent checkbox at the bottom is the part that makes the rest usable. A credit and reference check you can point to a signed acknowledgement for is a check you can run the same afternoon, rather than one that waits on an email nobody replies to.
Set the tenancy lengths you actually offer, and drop the guarantor field if you never take one. Landlords in regulated markets usually add the disclosures their jurisdiction requires above the consent section.
