About this template

Course feedback collected in the last week of term arrives too late to help anyone taking that course. Sent at the midpoint, the same questions become something you can act on, and the students who answered get to see the change, which is the only thing that makes them answer again next time.

The pace question is scored from far too slow to far too fast rather than bad to good, because pace is not a quality judgement and a five means something different from a five on clarity. Keeping the workload question as a similar spread stops it collapsing into a general satisfaction score.

The two open questions are where the actionable material is, so keep them last and keep them short. Add a question about a specific module if you are trialling something, and run the survey unlinked to student identity if you want honest answers about teaching.

Templates are a starting point, not legal or professional advice. Where a form carries consent, medical, or contractual wording, have somebody qualified review it against the rules that apply to you before you publish.
