About this template

Asking a customer for a testimonial usually produces something polite and useless, because a blank box is a hard prompt. Asking what the problem was before, what changed after, and then for one sentence gives you a structure that writes the quote almost by itself.

The permission section is what makes this form worth more than a nice email. Attribution and usage are settled in the same submission, so marketing can put the quote on a pricing page without going back to ask, which is the step that usually never happens.

The help text asking for a number in the result field is doing quiet work, since a testimonial with a figure in it outperforms an enthusiastic one without. Keep the anonymous attribution option, because a strong quote from an unnamed director still beats no quote.
