About this template

The awkward part of a wedding RSVP is that one reply covers several people with different meal choices and different allergies. This form handles it by asking for the party size and the names together, then asking for dietary requirements per guest in one field, which is far less work than a form per person.

Splitting the events into a checklist is what saves the catering budget. Guests who are only coming to the evening party should not be counted for a sit down meal, and asking directly is more reliable than inferring it from the invitation you think you sent.

The song and message fields do not affect logistics at all, and they are the reason people finish the form rather than abandoning it after the meal choice. Add a plus one question if invitations were not explicitly named, and set the accept option first so it is the default reading order.
