A workshop only works if the room is roughly at the same level, and the fastest way to guarantee it is not is to ask nobody. The experience question sorts a mixed signup list into sessions that make sense, and the four point scale from none at all to could teach it gives honest answers where a beginner to advanced scale does not.
The question about what would make it worth their time is the single most useful field on this form. Read the answers before you finalise the running order and you will find that half the room wants one thing you were planning to skip, which is cheaper to learn before the session than during it.
The equipment checklist prevents the classic opening twenty minutes lost to installations. Add a payment field for paid workshops, and use a rule so choosing online hides the lunch and travel questions entirely.
