About this template

Repair requests that arrive as a message saying the boiler is broken always need a phone call before anyone can be sent. This form asks for the category, the room, and the urgency as structured answers, so a job can be assigned to the right trade without a triage call.

The photo upload does more work than any other field, and the help text says so. A picture of a leak tells a property manager whether to send a plumber today or add it to next week's round, and it is the difference between one visit and two.

The three level urgency scale is written in terms of consequence rather than adjectives, so an emergency means a risk to health or the building rather than however strongly the tenant feels. Access and availability together let a contractor be booked in the same pass.
