What you need
A Deooch Forms account and a Claude plan that supports custom connectors.
How to connect it

In Claude, open Settings → Connectors and choose Add custom connector.

Paste the Deooch Forms MCP URL as the server address and leave auth set to OAuth.

Hit Connect — Claude opens our sign-in, and you approve it there.

Ask for what you want: "Create a customer feedback form with a rating and a comment box."
