About this template

Class bookings need three things settled before someone turns up: that there is a space, that the class suits their level, and that the waiver is signed. This form does all three, and the appointment field only offers sessions with room left so nobody arrives to a full studio.

Asking about level is not gatekeeping, it lets an instructor plan regressions before the class starts rather than improvising in front of the room. The pregnancy question includes a prefer not to say option because it is genuinely nobody's business, but it does change what a responsible instructor will cue.

The waiver is three specific commitments rather than a link to terms, which is both more likely to be read and easier to point at afterwards. Add a payment field for drop in classes, and drop the equipment question if you always provide it.
