About this template

A first session spent asking questions is a session the client paid for and did not train in. This form gathers the goal, the history, the injuries, and the availability in advance, so the first programme is written before the client walks in.

The injuries field has help text asking about anything that still bothers you occasionally, which surfaces far more than asking about current injuries does. Most people do not consider a shoulder that aches twice a month worth mentioning, and it is exactly what changes a programme.

Asking about sleep alongside training frequency is what stops a plan that looks good on paper and fails in week two. Add a payment field if you sell blocks up front, and keep the goal detail field required, since a vague goal produces a vague programme.
